Diabetes Resources for South Dakota State Employees

Did you know November is Diabetes Awareness Month? We did, and we’re here today to talk about the many opportunities available to South Dakota State Employee Benefits Program Members.

Nationally, over 30 million people have been diagnosed with diabetes, and another 84 million have prediabetes. South Dakota has seen a 66 percent increase from 6.7 percent in 2007 to 11.1 percent in 2017, the highest rate increase across the country.

The benefits and wellness programs have several resources available to state employees and covered family members. These are all available to members free of charge.

Better Choices, Better Health SD Workshop on Diabetes

The Diabetes Workshop Series is designed for adults living with type-2 diabetes, prediabetes, and individuals providing care to those with these conditions. Special emphasis is placed on monitoring, identifying symptoms, preventing complications, and more. The workshop consists of six classes, one per week, over a six-week period. Each class is two-and-a-half hours. Complete the workshop and self-report to earn 75 points.

Prediabetes Risk Test

Prediabetes is a serious condition in which a patient’s blood sugar is higher than normal, yet not high enough to be diagnosed with diabetes. People with this condition are more likely to develop type 2 diabetes.

Unfortunately, most people who have prediabetes do not know it, which is why we offer this seven-question risk assessment. Completing the test is an easy way to determine your risk of prediabetes. Completing the test and self-reporting the completion date will earn you 25 wellness points.

Lifestyle Health Coaching

A health coach can help you identify your goals and provide the tools and support to reach them. Choose from nutrition, physical activity, weight management, stress and more. To get started in a Lifestyle Health Coaching program through StayWell call 1-800-721-2749. Earn 25 points each month you participate, for up to 75 points.

HMP Conditions Management Program

Health Management Partners (HMP) provides Conditions Management programs at no cost. HMP’s health coaches and case managers can help members manage conditions through coaching, education, and guidance. Diabetes is one of the many conditions the program is designed to address.

Active participation may result in incentives including free office visits, free lab work and more! Members can earn 25 wellness points for each month of completion before the April 1 deadline.

Self-Directed Coaching

Anyone participating in Self-Directed Coaching can choose among topics including Control Weight, Eat Well, Move More, Smoke-Free, and Stress Less. Members will be required to complete all activities in the LEARN, PLAN, and USE section for the selected topic by the April 1 deadline to earn 40 wellness points.
