Conditions Management

The beneFIT Wellness Program provides Conditions Management programs through Health Management Partners to help manage conditions through coaching, education and guidance. The programs target conditions such as:

  • Asthma
  • Pain
  • Diabetes
  • Cardiovascular
  • Kidney Care

Active participation in a program may result in incentives including free office visits, free lab work, and more! In addition to the free incentives, you will earn 25 wellness points per month, up to 75 points, through your active participation. The beneFIT Conditions Management programs are available to all members* covered by the health plan:

  • State of South Dakota Employees
  • Retirees under age 65
  • COBRA members
  • Spouses
  • Dependents (Asthma and Diabetes only)

*Members who are newly diagnosed with one of these conditions and live in Brookings, Pierre/Ft. Pierre, and Rapid City are eligible for the Health Home Pilot Program. Please call DAKOTACARE at 800.831.0785 for referral into the program. You also have the ability to earn Wellness Points for your participation in the program.

Complex Case Management

Health Management Partners works on behalf of the State of South Dakota Health Plan to offer medical management services to state health plan members. One of the services available to members is Case Management.

Case Management is a collaborative process including the individual (person with health-related issue), their health care provider(s), and family/social support. The case manager will assist with everything from assessment and evaluation to coordinating and implementing the health plan. Case managers are advocates for the individual and can help navigate the complex medical system.

Case management may be beneficial in the following circumstances:

  • Active treatment of cancer
  • Transplant evaluation
  • Depression or chemical dependency
  • Traumatic injuries
  • Frequent hospitalizations
  • High cost medications or treatments
  • Neonatal Intensive Care Unit (NICU) admissions

Case Managers must hold a professional license. This list includes registered nurses, dietitians, pharmacists, and nurse practitioners.

Over half of the HMP case management team is certified through CCMC. HMP prides itself on being a center of excellence in all of our services and we are very proud of the certification through CCMC of our case managers. Case managers with CCM credentials have demonstrated the education, skills, and experience to deliver case management services.

This service is primarily telephonic and at your convenience. However, we are happy to accompany you to provider’s visits or on-site visits.
