The Three Wellness Qualifications Deadline is Approaching

The deadline for completing the three wellness qualifications is fast approaching! While the official deadline is April 1, we recommend that you finish all three by March 1. Completing your online health assessment, on-site health screening, and earning 100 wellness points by this date will help to ensure that your progress bar is maxed out by April 1.

If you cover your spouse, they must also complete the three wellness qualifications if you want to be eligible for the incentive. The spouse needs to create their own account at benefit.staywell.com. The employee can track their spouse’s progress under the My Progress tab. 

As a reminder, your progress bar needs to be populated to 100 points if you have individual coverage, or 200 points if you cover a spouse, prior to that April 1 deadine. Your progress bar cannot populate unless you have completed both the assessment or the screening.

What are the three wellness qualifications?

Complete these three wellness qualifications to earn the incentive:

Online Health Assessment - The Online Health Assessment will help you see how certain lifestyle havbits affect your health and well-being. It only takes a few minutes to complete!

On-site Health Screening - Receive valuable information to keep you healthy today and prevent serious health problems in the future. Screening dates are available at limited locations across the state. If you can't make it to one of these dates, you can use the Health Care Provider Form to meet this qualification. All you need to do is schedule an Annual Wellness Preventive Exam with your health care provider, print off the Health Care Provider Form, and have the clinic complete and sign the form. We recommend you submit the form yourself. Relying on your health care provider could jeopardize your chances of meeting that April 1 deadline.

Earn 100 Wellness Points - Choose activities to earn 100 wellness points. Log dates for dental or vision appointments, flu shots, or cancer screenings. Watch a webinar or participate in one of the well-being activities. Access a free personal health coach and get tips for healthier living. Earning points is fun, simple, and informative!

What's in it for me?

Participating in the beneFIT well-being program can lead to a great start to the new year. How, you ask? By focusing on maintaining and improving your health.

In addition, by completing the three wellness qualifications, you will qualify for the Low Deductible Health Plan for FY21 or you will earn the State's contribution for your Health Savings Account (HSA) if you choose the High Deductible Helath Plan for FY21.*

Failure to complete the three wellness qualifications will not only default you to the High Deductible Health Plan, but will leave you without any State contribution to your Health Savings Account.

In previous years, state employees on the High Deductible Health Plan received a partial contribution to their Health Savings Account whether they completed the three wellness qualifications or not. The policy was changed in order to provide incentive for high deductible health plan members to engage in the beneFIT well-being program. You can find a complete FAQ on the policy change here.

*If the employee covers their spouse, the spouse also needs to complete the wellness qualifications in order for the employee to earn the incentive.
