FY21 Life Insurance Information:

 

Basic Life Insurance:

The State of South Dakota provides each benefited employee $25,000 of Basic Life Insurance coverage. Employees cannot port Basic Life coverage but they can convert it if they leave employment with the state.

Supplemental Life Coverage:

You may choose supplemental life coverage levels between one to seven times your annual salary, up to $1,000,000 through MetLife. During your 30-day new hire enrollment period, you may elect one to five times your annual salary, up to a maximum amount of $400,000, without completing an evidence of insurability form (underwriting). If you request an amount six to seven times your annual salary, or any amount over $400,000, you will be required to complete an evidence of insurability form, which is subject to approval through MetLife. If you do not elect Supplemental Life during your 30-day new hire enrollment period, and then choose to participate in Supplemental Life, or choose to increase your coverage, you will also be required to complete an evidence of insurability form, also subject to approval through MetLife.

The MetLife Supplemental Life Insurance Plan is portable, meaning you may be able to continue the policy if your employment with the State ends, up to age 99.

To enroll for coverage under this Plan:

Dependent Life Coverage:

Employees who are covered under Supplemental Life Coverage may elect $10,000 Dependent Life Coverage.

To enroll for coverage under this Plan:

You must be enrolled in Supplemental Life Coverage-see above on how to enroll.

Accidental Death and Dismemberment (AD&D) Coverage:

Employees who are covered under Supplemental Life Coverage may elect AD&D Coverage.

To enroll for coverage under this Plan:

  • AD&D Coverage equals your Supplemental Life Coverage.
  • If you add AD&D to your supplemental life and elect dependent life, AD&D will automatically be added to the dependent life.
  • You must be enrolled in Supplemental Life Coverage-see above on how to enroll.
