Preventive Care

The plan covers the following items under preventive care when age and frequency requirements are met, if applicable. It is important to ask the right questions during your annual wellness preventive exam. To view a list of pertinent questions to ask your doctor, please consult the strong>Regular Checkups list from Health.gov.

Points can be earned for appointments occurring after July 1, 2021. Plan ahead!

We recommend calling now to ensure your appointment will be completed in time. It may take up to 10-12 weeks for the entire process to report your points. The well-being deadline is 4/1/22.

Earn 50 points for completing an annual wellness preventive exam.

Earn 50 points for completing one of the following preventive cancer screening exams

  • Breast Cancer Screening
  • Cervical Cancer Screening
  • Colorectal Cancer Screening

Employees that are the primary policyholder of the health plan can earn well-being points. Below is the process that occurs after your appointment to ensure your points will be populated. You do not need to submit a form or other information to earn your well-being points. If you want your biometric values to count towards the biometric screening qualification for Reward #1, follow the instructions in the next section.

  • Following your appointment, your clinic will submit a claim to Wellmark for processing.
  • Wellmark will process the claim.
  • Wellmark will submit a report to the well-being vendor, WebMD and your points will be populated.
  • Please note: It could take up to 10-12 weeks for points to display on the portal, depending on how long it takes your clinic to submit the claim.

You and your covered spouse can meet your Biometric Screening qualifications by completing and submitting the Physician Form during your Annual Wellness Preventive Exam. Click here to download it now. Follow the instructions and submit the form well in advance of the April 1, 2022 deadline. It will take up to 10 days after the receipt of your form, for the biometric screening to show complete on the beneFIT well-being portal.

Make sure all screening values are entered and both you and your physician have signed the form. Include your email address on the form. If you do not receive an email confirmation within 24 hours, resubmit your form.

Do I have to wait one year since my last annual wellness preventive exam?
No, you are eligible for one annual wellness preventive exam at any time during the plan year July 1, 2021 – June 30, 2022.
From the SPD:

  • One routine physical examination per plan year.
  • One routine mammogram per plan year.
  • One routine gynecological examination per plan year.
  • One routine pap smear per plan year.
  • One diagnostic screening for prostate cancer, including digital rectal examination and prostate-specific antigen (PSA) test, for:
    • Asymptomatic men aged 50 and older
    • Men 45 and older who are at a high risk for prostate cancer.

Will I Need to Submit the Form in Order to Meet the Biometric Screening Qualification?
If you do not attend one of the on-site biometric screening events, you will need to complete a physician form with your provider and submit to Total Wellness prior to April 1, 2022. Please save your notification of receipt from Total Wellness as confirmation that your form has been received and credited.

Will I need to submit the form in order to receive points for Preventive Care?
A form will not award points for well-being exam or cancer screenings. Points are awarded through the process described above. The process timeline is dependent on how long it takes your clinic to submit the claim to Wellmark and time needed for Wellmark to submit the report to WebMD. It may take up to 10-12 weeks for the entire process to report your points. The well-being deadline is 4/1/22.
