HSA Policy Change FAQ

We made the policy change in order to provide incentive for high deductible plan members to engage in the beneFIT well-being program.

By completing the three wellness qualifications by April 1, 2020:

  • You will qualify for the Low Deductible Health Plan for FY21

or

  • You will earn the State's contribution for your Health Savings Account (HSA) if you choose the High Deductible Health Plan for FY21
The three wellness qualifications are:

Yes, both the state employee and covered spouse must complete the three wellness qualifications by April 1, 2020, for the employee to be eligible for the incentive. Therefore, the state employee must ensure that their spouse has completed the qualifications in order to enroll in the Low Deductible Plan, or receive the State's contribution for their Health Savings Account if they choose the High Deductible Health Plan. The only exception is if your spouse was added to the health plan after July 1, 2019, then the spouse is not required to complete the wellness qualifications this year.

Yes. You can make an appointment with your Health Care Provider for an annual wellness preventive exam. Complete the Health Care Provider Form with your doctor during an annual wellness preventive exam. You and the doctor must both sign the form. Submit the completed form to Total Wellness, following the instructions provided.  We strongly recommend that you do not rely on the doctor’s office staff to submit it.

The Health Screening status will update by 10 business days following the submission of a valid Health Care Provider Form. It is highly recommended that if you are using a Health Care Provider Form, it is submitted by March 1. This allows for the 10-day processing period and to allow you to check your final completion status before the April 1 deadline.

Within 48 hours, an email confirmation will be sent to the email address that you provided on the form. If you do not receive confirmation, resubmit the form. Please allow 10 business days for the information to display as “complete” under the My Progress tab at benefit.staywell.com.
If your annual wellness preventive exam or any other doctor appointment occurred after July 1, 2019, you can ask your provider to complete the Health Care Provider Form using those results. Submit the Health Care Provider Form as instructed on the form.
Employees who were hired or added to the health plan after July 1, 2019 are not required to complete the wellness qualifications this year.
Login to benefit.staywell.com and click on My Progress to check your completion status. Points earned will not populate on your points bar until your online health assessment and on-site health screening have been completed. It is highly recommended that if you are using a Health Care Provider Form to meet your health screening qualification, it is submitted by March 1. This allows for the 10-day processing period and to allow you to check your final completion status before the April 1 deadline. Your green progress bar must show 100 points completed for individual coverage or 200 points if you are an employee with a covered spouse, to earn the incentive.

Individual Coverage: When your points bar shows 100, it means you have completed all three qualifications.

Employee with a covered spouse: If you cover your spouse, each of you need to have an account on the StayWell portal. You will both need to complete all three wellness qualifications to be eligible for the incentive. When your points bar shows 200, you and your spouse have met the three qualifications.
