You and your covered spouse can meet your Biometric Screening qualifications by completing and submitting the Biometric Screening Form during your Annual Wellness Exam (with blood work). Follow the instructions and submit the form well in advance of the April 1, 2025, deadline. It will take up to 10 days after the receipt of your form, for the biometric screening to show complete on the LiveWellSD Portal. Please note: Forms submitted between April 1, 2024 – June 30, 2024 will not be processed until the first week in July, therefore email and portal notifications will occur 10 days after that.

Make sure all screening values are entered and both you and your physician have signed the form. Include your email address on the form. If you do not receive an email confirmation within 48 hours, resubmit your form.
