Please be advised that as we work on finalizing our life events module, life events and family status changes will be conducted through an updated form until further notice.
As a reminder, employees are required to submit documentation along with their requests. This includes items such as a birth certificate for newly added dependents and marriage certificates for newly added spouses. Employees seeking to make a family status change will need to submit the required documentation to the Benefits Program with their request.
Once the Life Event module is completed, all family status updates and all family status updates and documentation will be submitted through the Employee Self-Service portal.
